About the role
We have an exciting opportunity within West Northants Council.
We are looking for an Adult Social Care Worker to join the Review Team. This team undertakes assessments, provides advice, and coordinates support for people who are:
- In hospital
- Discharged home with reablement services
- Transferred from hospital to settings such as residential Discharge to Assess placements or the Recovery and Independence Bed Unit
- The team's remit is to review and consider all options to meet Care Act eligible needs, with a strong focus on promoting and maximising independence, wellbeing, inclusion, and personal control.
As an Adult Social Care Worker, you will work alongside the Assistant Team Manager and Team Manager as part of a project team.
What will you be doing?
As an Adult Social Care Worker, you will undertake Care Act assessments/ reviews to determine eligible care act needs, undertaking other statutory assessments under the Mental Capacity Act, and other key legislation as defined in your statutory roles and function.

The successful candidate will be responsible for completing Care Act assessments/ reviews for individuals in receipt of a Direct Payment.

- Have up to date knowledge of legislation and demonstrable experience of delivering social care outcomes to people in need.
- Have good communication and IT skills.
- Able to prioritise, using a risk-based approach.
A degree or NVQ 4 or the ability to demonstrate knowledge gained through detailed practice and application for procedures in adult social care, is required.
This role is exempt from the Rehabilitation of Offenders Act 1974 and therefore subject to an enhanced DBS certificate, at our cost.
