About the role
Turnfurlong is an integrated health and social care model of care for people that need short term support upon discharge from hospital.
The 'Recovering Independence Bed' unit will be delivered by a range of staff, from both health and social care settings. Our team will deliver clinical care, rehabilitation support and care support, ensuring people experience the best quality of service they can, in a prompt and seamless way.
The services will provide a unique opportunity to work with a dedicated team, with experience from both elements of health and social care, to deliver vital intermediate care for people in Northamptonshire.
The team will provide quality care for our people, specialising in rehabilitation/reablement and sub-acute care for people discharged from acute hospitals.
As the focus of the centres is rehabilitation and reablement you will be working in an integrated multi-disciplinary team that includes senior nurses, staff nurses, unregistered staff, physiotherapists, occupational therapists, medical staff and social care colleagues.
Prioritise and direct the workload of team members, accordingly, ensuring any issues are dealt with as they arise and the health safety and welfare of customers and employees are responded to in line with agreed policies and procedure.
Undertake shift lead responsibilities on behalf of the service for ordering, monitoring, and administering medication to customers, ensuring prescribed drugs are used appropriately and safely in line with relevant policies and procedures.
Providing practical, personal and emotional care and support and to West Northants Council customers who may have a range of different health and social care needs.
Supporting the customer to maintain or develop social networks and interests, where required organising and or participating in activities of the customers choice. To always be mindful of treating customers with kindness, respect and compassion, that helps foster a feeling of happiness and contentment for customers in their home or place they visit.
What will you be doing?
Assist the management team by providing and recording information to help amend and update care plans so that changes in customers’ needs can be supported appropriately.
Carry out day to day, practical organisation of shifts in Turn Furlong Recovering Independence Bed Unit for people accessing our services.
To always show kindness, compassion and understanding towards customers.
To provide high quality and flexible support and care to customers in a range of services to enable them to achieve a level of independence to their own abilities.
To deliver a person-centred approach that meets each customer’s aspirations and abilities towards achieving their wellbeing, rights, and choices.
About you
Be experienced in Management and Leadership.
NVQ Level 2 in Health & Social Care or equivalent.
You will have the ability to manoeuvre customers as required, using differing levels of physical effort.
With a sound understanding of good care principles whilst maintaining straightforward, clear and concise records.
Good planning and team-working skills.
Awareness of sensitivity and discretion in all verbal and written information (Data Protection).
12 hour shifts of 36 Hours per week: You will be able to work flexibly, e.g. evenings, weekends and Bank Holidays,

This role is exempt from the Rehabilitation of Offenders Act 1974 and therefore subject to an enhanced DBS certificate, at our cost.
