As a Reablement Support Worker, you will support frail or vulnerable adults, who may have had a change to their health routine or lifestyle and are finding it hard to live independently.
What will you be doing?
- Undertake risk, manual handling and medication assessments in order to safeguard both staff and person.
- Maintain case records. Including written, verbal and using our bespoke IT system.
- Liaise with health care professionals. Maintaining good links and relationships.
- Assess for, request and install low and mid-level equipment.
About you
- A good understanding of Health & Social care through paid work, volunteering or personal experience.
- NVQ Level 2 in Health & Social Care is required.

The council is in the process of implementing a new pay structure and terms and conditions which will be applicable to this role. The outcome could result in a future variation to the advertised salary, and this will be discussed at interview.
