About the role
We have an exciting opportunity within the West Northants Council.
We are looking for an Adult Social Care Worker to support the Positive Outcome Review Team. This team are currently reviewing all current Learning Disability, Physical Disability, ABI, Autism and Mental Health clients in receipt of care and support from West Northamptonshire Council. The remit of the team is to review and consider all options to meet Care Act eligible needs that promote and maximise independence, wellbeing, inclusion, and control.
As an Adult Social Care Worker, you will work alongside the Assistant Team Manager and Team Manager as part of a project team and be responsible for completing reviews and Care Act assessments for individuals in receipt of a Direct Payment. You will also be responsible for completion of Care Act Reviews.
It is intended that the fixed term contract will end on 31st March 2025.
The successful candidate will work as part of the project team to achieve the best outcomes for the people of West Northants.

What will you be doing?
As an Adult Social Care Worker, you will undertake Care Act assessments/ reviews to determine eligible care act needs, undertaking other statutory assessments under the Mental Capacity Act, and other key legislation as defined in your statutory roles and function.

- Able to prioritise, using a risk-based approach.
This role is exempt from the Rehabilitation of Offenders Act 1974 and therefore subject to an enhanced DBS certificate, at our cost.
