The role is for an Assessment Enablement Worker within Adult Social Care working in our local Community Hubs where you are assisting people/patients of an assigned Primary Care Network (which are part of a group of GP practices), with the aim to remain active members of their local community. You will become part of the ICAN aging well collaborative.
The role uses a person-centred approach which includes listening and developing creative and personalised solutions to assist people to manage their lives independently for as long as possible.
There is a multi-disciplinary approach integrating services from health and social care and the community to help reduce isolation and improving general wellbeing and quality of life for that individual.
One Age Well ICAN worker will be working with people in Northampton, Royal Parks and MWEB and the other Age Well ICAN worker will be working in Daventry and Northamptonshire Rural.
We are looking for people who believe it is important to ensure people have choice and control over their own support needs focusing on their strengths and networks.
What will you be doing?
Make initial welfare calls to people within the assigned PCN as well as A&E discharges. Within these initial calls, identify their needs and follow up as appropriate.
Liaise with GPs and practice teams to identify people who are over 65years, frail or who have long term health needs and support
Make relevant referrals to local service provision such as but not limited to: OT assessments, GP, hospital, care provision and assessments, pharmacy, and medication reviews as well as community-based support groups, charities and volunteer groups.
Arrange, coordinate, and undertake initial home visits and assessments of need within a person's own home.
Undertake health checks such as weight, nutrition, blood pressure monitoring, temperature, and personal care needs.
Support people within the community to be able to remain as independent as possible within their own home and reduce the risk of crisis support.
Tasks may include but not limited to assessment and provision of low-level mobility aids, house clearance and cleaning, provision of items to meet basic needs such as emergency food parcels, supporting individuals with housing, finances, medication and follow up appointments.
Demonstrate the ability to recognise and respond appropriately when faced with a sudden deterioration or emergency, alerting the team or enabling a rapid response
Work alongside families and the person’s wider support network, ensuring their choices, needs and support within their own home are met as a long-term plan.
Maintain all records accordingly in accordance with local policies, The Care Act, Health and Safety Regulations and General Data Protection Rules.
Work autonomously with minimal managerial direction, making fair decisions within overall policy.
Document and monitor aspects of person's co-ordination and service delivery, supporting data collection and audit using the practices clinical system as well as organisational data systems
Maintain an individual workload whilst working cohesively alongside other Age Well partner organisations, supporting individual referrals as the team's collaborative approach
About you
You need to have
Experience of working with people.
Passion to promote people’s strengths and the ability to make a difference.
Some knowledge of legislation such as The Care Act 2014 and demonstrable experience of delivering social care outcomes to people in need.
Good communication and IT skills
Able to prioritise, using a risk-based approach
Be able to work as part of a collaborative team consisting of Adult Social Care, GP, s and Health colleagues, as well as collogues within Age Uk and Northamptonshire Carers.

The council is in the process of implementing a new pay structure and terms and conditions which will be applicable to this role. The outcome could result in a future variation to the advertised salary, and this will be discussed at interview.
