About the role
Our Reablement Support Officers/Rehabilitation Care Officers work with a number of people in the community each day. You’ll travel to the people’s homes to support them with regaining skills and confidence to enable them to become more independent, actively promoting their choice and dignity. It’s an incredibly rewarding role, every day and every visit is different! You’ll work with autonomy and independence but within a close-knit team, and you’ll have regular support from the Reablement Team Manager. You’ll also work closely with our health partners such as the intermediate care team.
You’ll case manage reablement journeys, from initial assessment through to discharge and support in the planning of their reablement goals. You’ll then visit them regularly to monitor progress and support them with day-to-day activities which could include personal care, meal preparation, mobility and accessing the community with the aim of enabling them to regain their. You’ll also support people by signposting them to other agencies where appropriate, for example Assistive Technology, Age UK, Northamptonshire Carers etc.
Day to day, you’ll see multiple individuals in their own homes, you’ll sometimes visit with a colleague but mostly you’ll work on your own. Your office base will be Chedworth Close in Ecton Brook, Northampton. We pay for whole shifts, not just individual visits, and pay 45p per mile, but also offer access to work vehicles.
What will you be doing?
Reablement West provides exceptional levels of Reablement support across West Northants in people’s own homes. We work to provide a fast, responsive service to individuals, who without immediate support, will be at risk of either being admitted to an acute hospital or care home or will be unnecessarily delayed from returning home. Our friendly and committed staff provide the highest level of Reablement support to vulnerable adults in West Northants.
We apply strength-based ways of working to enable people to regain their independence whilst assessing risks associated, which will include arranging equipment, connecting people to community resources and support, and increasing people’s skills and confidence.
Our approach:
- Focuses on a person’s strengths to promote and maximise independence and wellbeing.
- Rebuilds confidence after a spell of illness, deterioration in health, an injury, a hospital admission, or an acquired disability.
- Is different to traditional home care as it involves care providers standing back and encouraging the promotion of self-care skills that may have been lost.
- Supports people to regain or retain skills to enable them to manage with minimal or no support.
- Short-term and intensive.
- About setting and working toward meaningful goals.
- Aims to reconnect people with their communities to reduce social isolation.
- Can help to reduce the amount of care the person needs from carers and family.
- Reablement has a multi-disciplinary approach which means the teams work with one another to ensure your plan is person centred.
About you
You’ll have previous experience within a health or social care setting, and NVQ / equivalent level 2 in Health & Social Care. You’ll be confident working independently and have the ability to handle unexpected situations or challenges, being able to identify the source of a problem and find an effective solution.
A full driving license and access to your own vehicle is required because you will travel countywide.
