About the role
Community, Care & Support Worker (Reablement Service)
Please note this role is funded until 31 March 2027 and applicants must have the right to work in the UK until this date. Sponsorship is not available.
Our Reablement Service supports people within the community to remain safe, independent and well in their own homes. Working alongside health and social care professionals, we help individuals maintain their independence and continue living fulfilling lives within their local community.
The service operates 365 days a year, providing essential support to people when they need it most. Based from Ecton Brook, Northampton, our team delivers person-centred care and support across the local area.
What will you be doing?
This is a hands-on care role where no two days are the same. Travelling between people's homes is a key part of the role, so a full UK driving licence and access to a vehicle are essential. You will work independently within the community while receiving ongoing support from an experienced and friendly team.
Your duties will include:
- Providing personal care and support
- Administering and assisting with medication
- Supporting people with daily living activities
- Encouraging independence and confidence
- Promoting dignity, choice and respect
- Building positive relationships with the people you support
- Working alongside colleagues and healthcare professionals to deliver excellent care
This role focuses on providing practical care and support within the community. You will not be responsible for completing assessments or developing specialist reablement plans.
Shift Pattern
- 4 days on, 4 days off
- Average of 31.5 hours per week
- 2 morning shifts and 2 evening shifts during each cycle, on a 8 week rolling rota.
Shift Times
- Morning Shift: 7:00am - 4:00pm
- Evening Shift: 1:30pm - 10:30pm
Please note that the role includes weekend and Bank Holiday working.
What We Offer
- Mileage allowance of up to 55p per mile
- Full training and ongoing support
- Supportive and friendly team environment
- A rewarding opportunity to make a genuine difference to people's lives every day
About you

You will have:
- Some previous care experience (this does not need to be extensive)
- A caring and compassionate approach
- Good communication and interpersonal skills
- The ability to work independently and as part of a team
- A positive attitude and willingness to learn
- A commitment to providing high-quality, person-centred care
Previous care training or a Health & Social Care qualification would be advantageous but is not essential, as full training and ongoing support will be provided.
Essential Requirements
- Valid UK right to work that remains in place until at least 31 March 2027 (we do not offer sponsorship for this role)
- Full UK driving licence with access to a vehicle for work purposes
- Availability to work the shift pattern outlined above
