About the role
The role of the Wellbeing Advisor is Prevention and Early Intervention through working with individuals on a one-to-one basis in both community settings and their homes. In addition to 1-1’s, group work will be carried out in a variety of community settings.
You will be working with individuals across the 9 local area partnerships in West Northants, who require support with various areas of their lives, with the aim of reducing adverse health events and subsequent demands on health and social care by supporting customers with needs identified through the completion of a variety of validated assessments.
What will you be doing?
In this role, you’ll support individuals to improve their wellbeing by developing and reviewing personalised plans, offering guidance on behaviour change, and connecting them with local services and activities.
You’ll manage a varied caseload, carry out home visits where needed, and help deliver group sessions across local areas. Working closely with community partners, you’ll contribute to the delivery of targeted wellbeing initiatives and promote public health messages through events and presentations. You’ll also maintain accurate records, monitor outcomes, and ensure the safety and inclusion of everyone you support.
About you
You’re an empathetic and solution-focused professional with strong experience in supporting individuals with their health and wellbeing needs. You bring excellent relationship-building and active listening skills, along with a solid understanding of behaviour change, lifestyle management, and motivational interviewing. You’re confident in using assessment tools to identify areas of support, managing caseloads, and recording accurate case notes, while maintaining high standards of safeguarding and confidentiality.
You have strong IT skills and are confident using digital systems to manage information, communicate effectively, and support service delivery. As this role involves regular travel across West Northamptonshire, you’ll need to be flexible and comfortable working in a variety of community settings.
